Dr. Shaun Murphy (Freddie Highmore) goes through his normal bus routine to help him focus during his over stimulation as the hospital is in chaos around him. Nurse Dalisay Villanueva (Elfina Luk) checks on Shaun as Dr. Alex Park (Will Yun Lee) is calling out to his son, Kellan Park, (Ricky He) while he is having an asthma attack. Villanueva tells Dr. Morgan Reznick (Fiona Gubelmann) that Shaun isn’t responding and Morgan advises her to not yell and to give him the details of the surgery. After Villanueva tells him about the surgery, Shaun is able to focus and return to the surgery.

Dr. Claire Browne (Antonia Thomas) and Dr. Neil Melendez (Nicholas Gonzalez) resuscitate Chris Santos (Alex Weed) and Claire reminds Melendez that violating Chris’ rights could lead to them losing their licenses. Melendez assures her that she won’t lose her license, and that they should find a way to get Bob Cravens’ (William MacDonald) bone marrow to ensure this isn’t a waste of time. Nurse Deena Petringa (Karin Konoval) helps Kellan and Alex runs to him. Several people ask him when they can go home, and Alex orders everyone to shut up.

Upon being told that there is no nebulizer in the area, Alex puts together a makeshift one to help Kellan breathe. Kellan asks Alex if he used the tazer to break into the quarantine zone, and Alex assures him he merely threatened to use it. The makeshift nebulizer works, and Morgan updates Dr. Marcus Andrews’ (Hill Harper) on Dr. Audrey Lim’s (Christina Chang) condition. Andrews gives her the recommended treatment, but Morgan reports that the method failed on the previous victims of the virus. He reminds her that they got to Lim sooner and asks about Shaun’s breakdown. Morgan reports that he’s fine, and mentions that Alex is now in the quarantine zone.

Dr. Aaron Glassman (Richard Schiff) finds Lea Dilallo (Paige Spara) waiting for him, and he tells her that he has to wait for confirmation that his cancer is back. Lea is excited about the possibility of a false alarm, but Glassman insists that the diagnosis is inevitable. He lays out what the return of his cancer would mean, and Lea asks what they’re going to tell Shaun. Glassman tells her he’s not going to tell Shaun anything and invites her to go to the family and friends portion of the waiting room with him.

Melendez proposes a solution to get the bone marrow out of the quarantine zone, but Andrews tells him the idea won’t work. Andrews makes a suggestions that they operate in the janitor’s closet due to the low pressure and transport the bone marrow through the laundry chute and do the transplant in the laundry room. Morgan asks Shaun for the status of Santa Pete (Rell Battle) and Shaun answers as he has earplugs in his ears to block the noise of the flickering light. Shaun continues to be distracted by the light and Alex smashes the light. Shaun takes the earplugs out and reports that Viola’s (Camille Guaty) water broke.

Shaun tends to Viola as she screams out in pain. Viola asks if he’s going to fall apart again, but Shaun explains that the buzzing and most of the yelling is gone and that they should  be safe from the virus based on quarantine statistics. Kellan asks if he always says what he’s thinking, and Shaun tells him he believes it’s good to be honest. Kellan comments that he wishes that Alex was like that, and Shaun tells him Alex always gives his opinions. Viola understands that Kellan is talking more about feelings, and Kellan explains that he’s not really there for him. Shaun counters that Alex broke into quarantine for him, but Viola tells him that people need their parents for more than life and death situations.

Morgan stares at Tyler Durness’ (Dan Byrd) corpse and Lim expresses her condolences. Morgan checks her vitals, and Lim gives her treatment instructions before commenting that she always said she’d be working at this hospital until the day she dies. Morgan assures her they’ll be able to save her, and Lim advises her to take Tyler out of their sight moving forward. As she zips up Tyler’s body bag, she notices a tear in his mask. She reports it to Andrews, suggesting that the virus might not be airborne. However, Andrews tells her that they’re still under lock down until they can confirm it.

Kellan asks Alex if they can talk, and Alex asks if it’s urgent. He replies that it isn’t and walks away. Alex asks what’s wrong, and Kellan asks why he moved away. He tells Kellan that this hospital was a good opportunity, and Kellan tells him that he barely spends time with him when he does visit, asking why he bothers coming. As Esther (Patti Allan) tends to Santa Pete, Alex asks Cravens if he’s ready for the transplant. He asks the waiting room if anyone is comfortable assisting with the transplant, and Santa Pete tells him that Esther is a doctor. Esther clarifies that she is a retired vetrinarian and Alex declares that it’ll do.

Shaun determines that they need to induce Viola’s labor, but Viola insists that she doesn’t want the baby born in quarantine. He tells her that there’s no other option, and that this will be the first baby for both of them. Meanwhile, Lim’s condition gets worse and, Morgan and Villanueva rush to help her. Morgan reports to Andrews, and he asks her to hold up the phone so he can see Lim. Andrew gives her instructions, but Morgan makes a suggestion of her own. After some convincing, Andrews agrees to walk her through her suggested procedure.

As Viola screams, Shaun comments on the loudness and reports that he can’t see the baby’s head. He determines that they can’t remove the baby, and suggests a c-section. However, Petringa tells him a c-section isn’t possible. Lea asks Glassman when he’s going to tell Shaun, and Glassman insists that he’s not going to. He explains that Shaun nearly killed a patient the first time he found out about the cancer, but Lea reminds him that Shaun is his friend. Glassman replies that he’s Shaun’s mentor and it’s not Shaun’s job to take care of him.

Alex and Esther work to extract Cravens’ bone marrow as Melendez supervises through the glass. Cravens insists he deserves the pain, as he has been a terrible father. Alex assures him that he’s there now, but Cravens points out that he didn’t come back until Chris’ life was in danger. Claire reports that Chris’ vitals are dropping, and Alex speeds up the extraction. Shaun and Petringa attempt to shift Viola’s baby, but accidentally break the placenta wall. Shaun tells Petringa to page an gynecologist to supervise his first solo surgery.

Shaun and Petringa roll Viola up to the glass, and Shaun assures her that Dr. Garcia (Alison Araya) will be able to supervise the surgery. He fills Garcia in, and she gives him instructions. Shaun takes a moment to take in the scalpel he’s handed and begins the surgery. Claire rolls Chris to the laundry room and Alex finishes the extraction. As Shaun operates on Viola, her pressure starts dropping. Shaun is overwhelmed by everyone trying to talk to him at once and orders everyone to be quiet so he can concentrate.

Once everything is quiet, Shaun is able to pull the baby out but determines that he isn’t breathing. Shaun tries to revive to the baby, but Viola begins bleeding out. After debating where to focus, Shaun continues to work on saving the baby. Lea tells Glassman that he should tell Shaun, arguing that he deserves to know. Glassman accuses Lea of toying with Shaun’s emotions, insisting that she will eventually hurt him again. Alex wraps the bone marrow capsule in bubble wrap and drops it down the laundry chute. Melendez reports that the marrow is intact, but Cravens goes into cardiac arrest.

As Alex attempts to revive Cravens, Garcia urges Shaun to focus on Viola. However, Shaun declares that he believes he can save both Viola and the baby. Shaun gives Petringa instructions and asks the waiting room if anyone knows CPR. Kellan volunteers and Shaun has him work on saving the baby while he works on Viola. After understanding Shaun’s proposed method, Garcia gives him instructions. Alex attempts to operate on Cravens as Claire and Melendez successfully complete the transplant.

Glassman apologizes to Lea for what he said, and tells her that his diagnosis is meningitis, not cancer. He is not happy that he has to undergo surgery, but still insists he won’t tell Shaun. Lea calls him out for being a bad friend and mentor, but Glassman insists he’s protecting Shaun since he’s different. She reminds him that Shaun isn’t a child, but a very intelligent adult.

Shaun finds a way to stabilize Viola with Garcia’s instructions, and then goes to determine if the baby is able to breathe on its own. He determines that the baby’s lungs are drowning in fluid, and Melendez asks Morgan for an update on Lim. He asks if he can see her, and Morgan turns the phone around. Melendez compliments Morgan’s work, and Shaun works on reviving the baby.

Lea comforts Clifton (Colin Lawrence) as he waits for new on his son. Glassman tells them to shut up and Clifton is relieved to hear the baby’s cries. Shaun holds the crying baby in his arms as everyone applauds his work. He hands the baby to Viola, and Petringa calls it a Christmas miracle. Morgan runs upon hearing Lim’s alert go off, but Lim asks for water before complimenting Morgan’s work.

Shaun tells Santa Pete that he doesn’t have the virus, but that he’ll have to stay for post-op recovery. Esther offers to keep him company as he leaves the quarantine zone. Claire tells Chris he’s stable, and Chris thanks her. He asks if he can thank Cravens as well, but Claire reports that Cravens passed away after the extraction. Esther comes in and tells him that she was with Cravens when he passed. She tells him that Cravens had a lot of regrets, but that he was glad he was able to help save his life.

Alex sits with Kellan and explains that he left because he was a bad person and father. He explains that his father drilled in his head that he should never show emotion, and he left because he felt like a failure. Alex tells him he’s trying to quiet his father’s voice and tells Kellan he loves him. Kellan embraces him, and Morgan watches Tyler’s body bag being taken away as she steps outside the hospital.

As everyone leaves the hospital, Kellan embraces his mother, Mia (Jennifer Birmingham Lee), telling her he helped deliver a baby. Alex tells her that Kellan ws great, but admits that he lost his own patient. Mia embraces him and the three leave the hospital together. Melendez visits Lim and wishes her a Merry Christmas. Lim admits she won’t forget this one, and Melendez tells her that he didn’t want to lose his bourbon-drinking buddy. Lim tells him that her bourbon-drinking days are behind her for the moment, but Melendez tells her that he’ll wait for her.

Lea comments that Shaun must be exhausted as she greets him, and he tells her he’s hungry as well. She suggests they go get pancakes with Glassman, and Shaun apologizes for not being able to take Glassman to the doctor. He asks how it went, and Glassman tells him that he’s going to need surgery for meningitis. Shaun calls it good news, as meningitis isn’t as bad as cancer. Lea asks if Glassman can hug him, and Shaun agrees as Glassman tells him he’s proud of him.
