Dr. Ramsey Rosso (Sendhil Ramamurthy) is at Central City Hospital looking under the weather. He’s been away for a little bit and lies saying that he’s meeting a friend, but actually goes to break into a case full of blood in the lab.

Barry (Grant Gustin) and Iris (Candice Patton) are discussing the events of how December 10th will go with Cisco (Carlos Valdes) and Killer Frost (Danielle Panabaker). Barry says that he has to accept that there are others out that that need to be saved. Cisco is upset about Barry dying and isn’t set on not trying to find a way to save him. Barry wonders how he can convince him to let him die, but he gets a CCPD call right after and has to leave. Leaving a sad and solemn Iris still sitting on the couch.

Joe (Jesse L. Martin) informs Barry that someone broke into the facility at the hospital and took ten liters of blood. He wonders if it’s meta related and thinks that it is probably just some kids pulling a prank. Joe asks if Barry is okay and he tells him that Cisco’s attitude with the Crisis is hard. Joe gives him some quick advice and Barry thanks him for giving him a way to get through to Cisco.

Ramsey is injecting himself with a needle as he records his findings with the black blood. He says he needs more blood to create a permanent cure to the HLH and adding Romero’s blood to the new blood should work. He starts the process and we see flashbacks of him and his mother (Meera Simhan) while she was sick.

Barry texts Cisco to meet him, suggesting that they can do more to save a life by saving Ramsey Rosso’s life. Cisco doesn’t immediately hop onto the idea. He asks Barry if they’re supposed to just spend their time with him trying to cure someone of cancer. But Barry tells him that he needs him to get on board with this. Cisco says that their chances are slim, but Nash Wells (Tom Cavanagh) comes up the side of the balcony and tells them that it might not be so slim. He introduces himself and assumes that Barry is The Flash. He admits that he bugged Cisco. He says he needs an anti-vibrational crypto-circuit to complete his work and they need help with their mission. So, if they’ll build him his circuit he will help them with their issue. Cisco calls red flags on this, but Barry says that it usually means something when a Wells shows up.

Iris comes into Ralph’s (Hartley Sawyer) office with news on the woman he’s been working on. She brought him a lead that says a security man saw Sue Dearbon. Ralph barely acknowledges her and she stands awkwardly waiting for more before she decides to leave.

Ramsey has completed the blood serum and injects it into his arm. It starts to encase his arm as it’s not working. He falls to the ground.

Wells tells Barry and Cisco that McCullough Tech has a serum capable of healing organic tissue no matter what the harm. They question the serum, but Wells says that they stole it from the Dominator home planet. When Barry leaves to secure the place Cisco asks if the serum cure someone from antimatter and Wells tells them it can. Barry returns and they go inside the building where he can’t use his powers. Some guards catch Barry and Cisco, but Wells disappears. Wells throws a sound device and cut the lights, successfully taking the guards down.

Iris asks Ralph about the Dearbon lead again, but Ralph has decided he has given up on Sue. Iris tries to encourage him, but he just makes a remark telling her that if he were her than he would be spending a lot more time with her husband than on him. She looks appalled and he leaves her there.

Barry finds the case with the serum in it. Just when they’re about to open it an alarm goes off that Cisco discreetly causes and Nash and Barry go to fix it. Cisco opens the safe and steals the serum so when Barry returns to open the safe it’s missing. Barry thinks McCullough must have stolen it. Nash says someone is coming and gives them devices that pop them back to the lounge. He immediately starts asking about them helping him with his device, but Barry says that’s off because they didn’t find it. But Wells doesn’t take no for an answer.

Ramsey is frustrated and throws his things around the lab. He flashes back to when his mother told him that she wasn’t going to fight the disease.

Barry tells Cisco that Nash isn’t going anywhere until he gets his circuit. Barry thinks Wells is lying to them. Barry sees a case with -15 degrees on it like the one they saw at the lab and asks Cisco what it is. Barry can tell that his response is a lie, so he opens it to find the serum. Cisco tells him that if he wants it he’s going to have to go through him. Cisco says that he’s saving his life, but Barry let’s him know that he made a call and that’s saving a life. Cisco is upset and asks what does that give him to look forward to. He doesn’t want a life without his best friend, but Barry says he has to get on board. He wants Cisco to understand making the tough calls and running STAR Labs after he’s gone. Cisco says he’ll steal again to save Barry’s life and Barry thinks that maybe he made the wrong choice. Cisco gives the serum to Barry before leaving the room.

Barry goes to Ramsey’s lab to give him the serum. Ramsey says he’d given up hope. He tries to offer it back to Barry, but Barry tells him that there is only enough for one person and Ramsey deserves to have it. He can help more people.

Joe visits Ralph to see what’s going on with him when he tells Joe that he doesn’t see why he should help Sue when he can’t even save Barry. Joe explains that he can’t always save everyone, but he can save someone and that’s Sue for Ralph.

They give Wells the device and he quickly starts to criticize it. He takes it anyway. Cisco tells Killer Frost that he thinks his and Barry’s friendship exploded but he still wants to try. She tells him that he can’t save someone that doesn’t want to be saved.

Ramsey tests the cure that Barry gave him, but it doesn’t work. Ramsey thinks there is no escaping it until he realizes something. He thinks he has to kill in order to release the substance that he needs and in order to do that he has to be afraid. He stands and his eyes begin to turn black.

Multiple alarms go off at the hospital because a man is attacking patients. The Flash and Frost go to the hospital where they see dead bodies all around. Barry hears a scream and runs off. Ramsey is killing someone else when The Flash throws him away from the body. Ramsey tells him that the cure didn’t work, so he’s doing whatever it takes to survive. He believes that he was too far gone for the cure to work. He throws The Flash across the room. The body Frost is looking over grabs her in a chokehold and she has to fight them off. Some of the attacked rise and try to attack again, but The Flash hits them with a lightning bolt. Frost puts them behind a wall while Ramsey has another victim in a chokehold. He tells them he has found a way to defeat death. Barry says he needs to find meaning in his life, but Ramsey thinks he’s found it – life eternal. He attacks the doctor who couldn’t save his mother and jumps out the window. The body is he left behind fall into a puddle of blood.

Barry has everyone gathered in the lounge as he tells them that they have to stop Ramsey because he’s still lost. They try to tell him otherwise, but Barry says he has to stop them. Ramsey is turning people into human blood banks. Ralph give a solemn “Happy Halloween” and leaves. Iris follows after him.

In the hallway Ralph apologizes for the way he reacted to Iris trying to help him with his Sue Dearbon case and tells her that he will continue to pursue it. Iris says that Sue is lucky to have detective like him on the case and he says it’s not like he’s looking to marry her before he leaves.

Cecile (Danielle Nicolet) says that she and Joe should probably head home, but Joe wants to stay for a little while. She leaves and encourages Barry to talk to Joe. Barry goes to Cisco and they share a tender moment. Cisco is still sad, but he says he knows he’s doing it for all of them because that’s what it means to be a hero.

Barry joins Joe on the balcony. Barry reveals to Joe that he just wanted to do some good by Ramsey before he died, but Joe tells him he’s done plenty of good as The Flash. Joe says he would have lost their family a long time ago without The Flash. He starts to break down as he tells Barry that he deserves better than having to die in order to save people. Barry starts to cry as he says he’s grateful for everything in his life, but he’s most grateful for Joe because he would have none of that if Joe didn’t show him the will to keep going. Joe breaks down saying that he’s not ready for this and the two embrace as they continue to sob on the balcony. He tells Joe, “Even when I’m gone I’ll still be here.”

Wells goes back to the sewer where he uses the device Cisco made him. A hologram pops up of The Monitor and he sees it walk through a wall. Wells takes out a pick saying, “So this is where you’ve been hiding.”
