Dr. Shaun Murphy (Freddie Highmore) visits Dr. Aaron Glassman (Richard Schiff) and declares that he wants them to be friends again. Glassman reminds him that he asked for space, but Shaun replies that he needs a friend more than he needs space. Shaun calls Glassman a good mentor and friend, but Glassman tells him that he’ll need a new mentor, as he’s dying. Glassman tells him he has twelve to eighteen months and asks him not to tell anyone. Shaun attempts to brainstorm other possibilities for his illness, and tells Glassman he should get a second opinion. Glassman tells him he’ll get a second opinion to stop him from talking.

 

Dr. Audrey Lim (Christina Chang) and Dr. Jared Kalu (Chuku Modu) examine their patient, Caden Hauley (Donie MacNeil), who declares that he’s drunk. Jared assesses that his foot is dying and Lim resets his leg. Caden’s friend, Blake (Graham Patrick Martin) asks Dr. Alex Park (Will Yun Lee) if he’s going to be okay. Parks asks Blake for details on Caden’s condition, and is suspicious of his story. In surgery, Caden asks if he should be asleep for the operation, but Dr. Neil Melendez (Nicholas Gonzalez) tells him that his intoxication affects the anesthesia. Caden’s blood pressure starts dropping and Melendez declares they need to stop surgery until they figure out the true nature of his condition.

 

Shaun works to find a solution for Glassman’s diagnosis and discovers that his toy scalpel is missing. Glassman tells him that he had a colleague look at the scan and gave him the same diagnosis, and urges him to go back to work. Park tells Jared to be reasonable and demands that Blake tells them the truth about Caden’s condition. Jared plays the good cop to Park’s bad cop and gets Blake to admit that Caden ingested laundry detergent as a fraternity pledge. As they resume the surgery, Dr. Morgan Reznick (Fiona Gubelmann) comments that Caden shouldn’t breed. Melendez is called to another room and puts Shaun in charge of closing up the surgery. However, Shaun freezes and declares that he lost something.

 

Shaun searches for his scalpel in Glassman’s office, and advises him to get a special kind of imaging. Glassman refuses, and Shaun is unable to find his scalpel. Jared tells Melendez that Caden is bleeding internally, and Shaun visits Jessica Preston (Beau Garrett). He informs her that Glassman is dying, and asks her to sit down. Shaun asks her to convince Glassman to get the imaging, and she confirms that he wants her to manipulate him.

 

Melendez talks to the team about Caden’s issue, and they debate what caused it. Morgan suggests that the issue is their fault, arguing that Shaun lost his focus during surgery. Dr. Claire Browne (Antonia Thomas) defends Shaun, and Melendez states that complications happen before giving the instructions. Melendez visits Glassman and asks why Shaun was distracted. He tells him that Shaun got some bad news, and Melendez questions what the news is. Glassman tells him about his diagnosis, and Melendez gives his condolences before asking if there’s anything he can do. After Glassman says there isn’t, Melendez tells him that he should of told him and that he shouldn’t have told Shaun.

 

Claire suggests that drugs could be involved in Caden’s condition, but Morgan accuses her of not wanting to admit that Shaun is at fault. Jared tells them he’ll find out what drugs he’s possibly on, believing that Blake will be honest with him again. He patches up Blake’s hand, and asks to draw some blood. Blake agrees, and Jared tells him that Caden will die if they can’t figure out the cause of his condition. Jared hands the blood sample to Park, who accuses Blake of being on drugs and threatens to give his blood sample to the cops. Blake admits that Caden was on molly, and tells Jared he would have told him the truth if he asked.

 

Jessica asks to speak to Glassman, and he assesses that Shaun told her about his diagnosis. She tells him that she wants to be there for him, and admits she’s mad he didn’t tell her. Glassman angrily tells her he wants to be left alone, and Jessica points out that telling Shaun wasn’t a good idea. He assesses that people do stupid things in reactions to diagnoses and concludes that Shaun asked her to talk to him. Jessica tells him he can’t stop fighting, and Glassman gets angry with her for bringing up Maddie. She apologizes and tells him to get the imaging because Shaun hasn’t accepted his diagnosis.

 

Dr. Marcus Andrews (Hill Harper) asks Melendez about the complication with Caden. Melendez argues that complications happen, and Andrew asks about him not being in the room. When Melendez argues that he’s done it before, Andrews tell him he hopes Caden pulls through. Shaun and Jessica go with Glassman to get the imaging, and Shaun asks why he listened to Jessica and not him. Glassman refuses to answer, and the doctor (Anjali Jay) tells them that the initial diagnosis was wrong. She tells him that the diagnosis is worse than they thought and that Glassman has three to four months to live.

 

Morgan tells Blake that the drug treatment didn’t work, and Jared interrupts her to suggest that Caden has a bacterial infection. Claire gets the test results and Andrews questions her about the case. She tells him that complications happen, and Andrews tells her Melendez told him the same thing. Claire insists that the surgery went normally, but lets slip that Shaun was in charge of the closing.

 

Glassman sees that Shaun has turned the conference room into a scientific lab, and Shaun explains that there are other explanations for his condition. Shaun refuses to accept Glassman’s pleas to accept his diagnosis, arguing that imaging is unreliable. Glassman tries to get Shaun to stop and screams at him to stop. Shaun insists he’s helping him, but Glassman tells him that he doesn’t need his help and just wants to enjoy his remaining time. Glassman tells him that fighting is futile and stupid, and asks Shaun to come with him. Caden takes a turn for the worse, and Melendez declares that they have to resolve the issue quickly.

 

Shaun and Glassman ride a carousel together, and Shaun asks if this is fun. Glassman tells him that he and his daughter used to do this together, and flashes back to him and young Maddie (Audrey Wise Alvarez) dancing together. They playfully argue over who loves who more, and Glassman tells Shaun that he always knew it would be the last thing would remember when he dies. Shaun tells him about his missing scalpel, and suggests that he will return to the carousel after Glassman dies. He asks if his memories of his daughter are enough for him, and Glassman replies they’re not even close. Shaun gets a call and asks if Glassman needs him to listen to him more.

 

Claire and Jared refuse to admit that surgical error could be the cause of Caden’s condition, and make other suggestions. Melendez asks Shaun to make a suggestion, since he is the one in trouble. Shaun flashes back to the surgery and determines that it’s his fault before announcing that he has to go to the bathroom. He visits Glassman and suggests a procedure that can save his life. Glassman reminds him he doesn’t want to think about his diagnosis, but Shaun insists that there’s still hope. However, Glassman tells him there’s no hope for him. Shaun pulls up next to him and continues explaining his idea, declaring that he can’t die too.

 

Morgan suggests reporting Shaun’s error, but Claire argues that she just wants Shaun out of the way. As Shaun returns, Park supports the hospital protocol. Jared argues that they should cut Shaun some slack due to him finding out about Glassman’s diagnosis, and Claire explains to Shaun that Melendez told them. Shaun says it’s okay, as he is confident that Glassman is going to be okay. Claire argues that Shaun’s consequences would be worse than the rest of them, and Jared suggests that he take the fall for Shaun’s mistake. Shaun argues that they would fire Jared as well, and Melendez urges them to get back to the medicine. Melendez tells them they won’t have to report anybody if Caden survives, and Shaun still struggles to find his scalpel. Melendez tells Shaun to return to the group and help find a solution.

 

Andrews asks Allegra Aoki (Tamlyn Tomita) if her opinion of Shaun has changed since he arrived at the hospital. He reminds her that Glassman promised that he would resign and Shaun would be fired if Shaun were less than excellent. Aoki insists that Shaun has exceeded her expectations and that her opinions of Glassman remain the same. Shaun uses his textbook memories to suggest a solution, but Melendez and Park argue against it. Claire argues that doing something is better than doing nothing, but Park continues to argue. Morgan defends Shaun’s idea, but then tells him that she still thinks he shouldn’t be working there.

 

The team attempts Shaun’s idea, and Shaun asks if he can perform the operation. He argues that it’s his mistake if Caden dies, and that Melendez shouldn’t have to live with it. Melendez tells him that everything that happens is his responsibility and tells Shaun that they’ll do it together. Later, Shaun finds the toy scalpel in his backpack as the team packs up. Melendez informs them that the operation worked and that he’s proud of them. He tells the group that drinks are on him, but Morgan questions why they’re celebrating. Melendez tells her that getting lucky is a cause for celebration, and warns her that she needs to have allies in her corner for when she makes a mistake in the future.

 

Shaun tells Glassman that he found his scalpel, and realizes that he took his advice. Glassman explains that Shaun was right about his diagnosis, and that this form of cancer could potentially be treatable. Shaun hugs him and tells him that he’s going to live. Glassman tells him to go celebrate, but Shaun announces that he has to report his mistake to Andrews. Jessica reminds him that reporting it would put both his and Glassman’s job in jeopardy, but Glassman tells him that the decision is up to him. Shaun assures Glassman that he’s not trying to hurt him as the walk to Andrews’ office, and declares that he loves him. Glassman replies that he loves him too, and Shaun replies that he loves him more as they step inside Andrews’ office.
